The T.A.R.D.I.S. comes down, Sherlock and John come outside and meet The Doctor.
The three of them parlay, or whatever plot device it is that gets Sherlock and John inside the T.A.R.D.I.S.
They go, and solve something that just makes Sherlock ITCH IN HIS BRAIN – say, figuring out who Jack The Ripper was.
They return to 221B Baker Street, their adventures over.

Sherlock thanks The Doctor, in his own itchy way, and starts to ascend the stairs to 221B Baker Street.
John takes a step out, then turns and re-enters the T.A.R.D.I.S.
John becomes the new companion for a time, say, a season or two. Adventures are had, Big Bads are stopped, Jack Harkness blah blah whatever.
At the end of that time, The T.A.R.D.I.S. lands in front of 221B Baker Street, and…
…John follows Sherlock up the stairs…
